How to Start

 

Follow the steps provided below to send your case to us.
If you are a new client, please contact our office at 604-321-4080 to create an account with us first before downloading the form.


STEP 1
Please download and print the prescription form. Fill out both sides of the form and cut the form in half. Keep the Client Copy for your record and send the E-Dent Copy to us with the model.



STEP 2
Cases from inside the lower - mainland:
E-Dent provides pick- up and delivery service for your convenience. Please call our office for further instructions.

Cases from outside the lower - mainland:
Please ship the model with the E-Dent Copy to the following address:
